If you’ve seen a colleague, a team or a volunteer do something special recently or perhaps you want to recognise them for doing great work every day then just drop us a nomination. It’s easy.

 

The form is below. Doesn’t take long, just fill it out and return to Head Judge  Jodie.Fry@nhs.net – and your nomination is in the running.

 

A member of the Senior Leadership Team will come to their work area to make a lovely presentation with their team - and they get £50 of shopping vouchers too.

Guidance Notes for nominations,

People

Think about how the person/team you've nominated has helped a patient, colleague or visitor either in particular situations or their general approach. Why did what they do make a difference? What do you think (or know) the people or person they dealt with feel or do as a result?

 

Reputation

How has the person/team you've nominated helped improve the hospital or Trust's reputation or helped their department or team's reputation in the Trust?

 

Innovation

Guidance notes: Has the person/team you're nominating come up with new ideas or ways of doing something to help improve a situation or change something for the better. Were they asked or did they just get on and do it? Can other departments/teams learn from what they've done? 

 

Dignity

How has the person/team's approach improved dignity and well-being for patients. Even if this person doesn't work directly with patients, think about how what you're nominating them for has helped keep high standards of patient care in the work they do. Did they behave with dignity and professionalism themselves? If yes how?

 

Excellence

This is your chance to include anything that you couldn't put in the other boxes and tell the judges why this person/team really deserves the Award
