As part of completing a Site Specific Information (SSI) form applicants are asked to complete a section on authorisations.
To undertake research at WAHT we expect authorisation to be gained from any departments that will be providing resources for the study. Most commonly these will be Pharmacy, Radiology and Laboratory Medicine but there may be other departments providing support for the project.
Rather than requesting researchers gain signatures on the SSI we have produced a set of pro-formas that collect the necessary authorisation and hold a record of the agreed resources to be provided.
The pro-formas should be completed by the researcher and sent to the appropriate named individual shown on the pro-forma. The form should either be signed by them or attached to an authorising email.
The pro-forma(s), and where applicable, the email should form part of the local R&D submission.
