Commenting today (21 July 2015) on a report by the patient watchdog “Healthcare England” regarding people being discharged early from hospitals, Karen Croker, Director of Operations at the Trust said:“From the moment patients arrive at Weston Area Health Trust our aim is that staff will work with them, their family, or carers, to set appropriate expectations around their length of stay, and we discuss any additional support patients might require on discharge.“Our protocols are that we only discharge patients that are medically fit, and when we’re confident appropriate arrangements have been put in place. If additional support is required, we work with patients, their family, and external agencies, to ensure a smooth transition from one place of care to another.”
