What are digital assessments?

Digital assessments help you tell us about your health in between appointments. We will send you a text message with a link when you have a new digital assessment to complete. These online forms ask you about your symptoms, physical health, and quality of life. They can also capture information about mobility, language, or any other additional special needs. Your care team will review your answers online which will help them prepare for your appointment in advance.

Why are we introducing digital assessments?

It is not always easy to tell your care team how you have been feeling over time. Regular monitoring of your health at home helps us:

  • Better understand your health before and after your appointments  
  • Give you a more active role in your health care  
  • Improve our understanding of treatments and any side effects  
  • Use data to make better decisions around delivering care

Where are we using Digital Assessments?

Digital assessments provide your clinical team with real time valuable information to enable them to support your care journey. It also provides information which you can access to track your health and help us improve your care/experience that will lead to a better all-around health service for the future. Where digital assessments would be beneficial in supporting your care journey, other clinical teams will introduce this service in future.

What do I do if I am unable to complete questionnaires on my mobile device?

It is possible to use the link on the tablet as well as your phone. If you are unable to complete an online form for any reason, make contact with your clinic using the contact information on your appointment letter and the clinic will make alternative arrangements for the assessment to be completed with you, so your information can still inform your treatment.

If you would prefer not to be contacted by phone or email for this purpose, please contact your clinic and update your contact preferences and we will automatically exclude you from these communications in the future.
