Discharge to Assess – what is it?
A new and exciting service which has been launched to help reduce your stay in hospital, and get you back home quicker.
Once you’re at home, your ongoing care and therapy needs will be assessed there, in a familiar surrounding, by a fully qualified Occupational Therapist or Physiotherapist.
How does it work?
Discharge to Assess is being delivered with some of our partners in the local community. The community team consists of occupational therapists, physiotherapists, rehabilitation support workers and social workers.
We work with you to identify what your on-going care and support requirements are so we can assess that you are able to recover at home safely and whether you require an on-going package of care.
How long we support you depends on your needs which will be assessed, usually on the first day you get home from hospital.
What can we help with?
We can support you with a range of daily living tasks, which might include help moving around the home, washing and dressing, or preparing drinks and meals.
When we meet you to complete our initial assessment, we can identify what tasks you will need help with and at what time of day.
